docs(issue-21): review identity and the remote-reviewer rule in the loop skill and the design doc

- loop SKILL.md: --review-isolation and --remote-review in the option table,
  a paragraph on review identity (snapshot, snapshot diff, stale re-review,
  pre-promotion re-check, report-<jid>.identity.json with evidence, nested
  repositories refused), and the remote-reviewer rule in the Issue #18
  remote-session section.
- design doc: the §2-C reviewer sentence describes the implementation; §5
  marks item 5 as loop B and says plainly that item 7's acceptance-case tests
  are not written yet; the §7 loop row states the remote-reviewer limit and
  that snapshot sharing is out of scope; a compatibility note for the
  --worktree/--base + remote-reviewer combination.
