GodopuandClaude Opus 5 9a651ccc71 docs(issue-5): record atomic attribution and correct my earlier claim
The state-transition spec gains the rule that attempt identity lives in the
record, because a precondition can only be atomic with the commit if the
transaction can read it under its own lock, and checking attribution beforehand
leaves a window a re-dispatch fits through.

The operations note gets the same, plus the reason it is an opaque token rather
than the iteration counter: iteration already means the loop round, so comparing
a dispatch attempt against it is vacuous when they agree and wrong when they
diverge.

It also marks my earlier section as partly withdrawn rather than quietly editing
it. That section argued a record schema change was unnecessary because the
attempt guard lives in the evidence layer. That was right about comparing
evidence to a dispatch and wrong about atomicity, and the review proved the
difference by execution. The narrower point it made -- why not to reuse
iteration -- still holds and is kept.

Also records why the subscriber now deduplicates on the intent identity: a
replay arrives with a fresh wire sequence, so the monotonic guard cannot see it.

GodopuandClaude Opus 5 0e74aa1e4e docs(issue-5): record the evidence requirement, the audit lock and the ack rule
Three contract points the round 2 fixes established.

An idempotent re-receipt is an ACK in the table, so a caller must treat it as
success. Only a genuine refusal is non-zero, and an invalid status stays the
usage error it always was. That distinction is what keeps the wrapper's approved
path from aborting under set -e.

Completion attribution now requires evidence to be present, not merely
non-contradictory, and names where it may be read from: a collector envelope
beside the payload, or a non-native source's own payload. Never from inside a
native Herdr payload, because MAM fields are forbidden there and one appearing
anyway is not evidence of anything.

The audit mirror's ordering guard is documented as a locked read-compare-write,
with the reason a bare sequence number was not enough, and the note that the lock
is deliberately taken outside the registry transaction.

GodopuandClaude Opus 5 3b134e3358 docs(issue-5): record the operator-authority transitions and the review fixes
state-transitions.md §3 gains the two rows it was missing. They are named
transitions, not exceptions: re-dispatching a finished job to the next iteration
and finalising an exhausted one are things the wrapper legitimately does, and
they are gated on operator authority so an agent still cannot walk back its own
terminal. The table also now states that a status may only be set by the request
that owns it, which is what closes the metadata-merge bypass.

phase2a-shadow.md documents the authority split, the CLI's new non-zero exit on
a refused write, and why the audit mirror compares transition_seq. Two reviewer
observations are carried forward as 2b work: `started` does not yet verify the
assigned session, and stop_session's unconditional cancel is now refused on an
already-completed job, which is the table behaving as designed but worth a
release note.

GodopuandClaude Opus 5 17a2dcd988 docs(issue-5): phase 2a contracts, scenarios and task checklist
Records what phase 2a actually delivers and, as importantly, what it does not:
the shadow runner decides and logs, and nothing commits an inferred completion
until 2b.

state-transitions.md gains an implementation mapping so each contract row names
the code and the test that hold it up, with the inferred-completed row marked as
judged-but-not-committed.

phase2a-shadow.md is the operations note: the rollback switch, the lock
discipline and why transition_seq exists, the herdr wait rules that are easy to
get wrong (milliseconds, always name --until, errors on stderr, a timeout is a
healthy answer), the fail-closed decision order, and the 2b handoff.

task.md records P2A-00..04 with commit evidence, including two corrections to my
own plan: pick_pending was already a correct swap rather than a hole, and the
engine had to live with the registry writers because putting it in lib_py/loop
would have made delegation import a sibling and mix trees under a frozen loop.
